You will receive a text link to complete your registration three days prior to your appointment. Completing the registration in advance of your appointment will speed up your office visit with Foot and Ankle Associates!

If you are unable to complete your registration ahead of time, please plan to arrive early to complete it at the office. If you do not receive a registration link 3 days ahead of your appointment, please call our office to confirm your mobile number.

Please bring your insurance card, any referral paperwork and a method of payment to your appointment.
