We are thrilled that you have chosen our podiatry team for your foot and ankle care. We will do our best to provide you with the most up-to-date and comprehensive podiatry care available. We have a total commitment to keeping your feet healthy – and keeping you happy.
Our new patient registration is now fully digital! To maximize your time with us, we ask that you bring the following to your first visit:
- photo identification
- medical insurance card(s)
- written referral (if required by your insurance company)
- prior medical records and x-rays (if applicable).
In addition, if you have not been seen by your physician in the last three years, please arrive 30 minutes early to complete the registration process on our electronic kiosk.
You may login to the patient portal (at the top of this page) and complete your medical history prior to your appointment to expedite the check-in process. For questions relating to your visit, please call us today or visit the patient portal.
