Dr. Blake Wallace grew up on California’s Central Coast in Atascadero. He attended California Polytechnic State University, where he graduated with honors in Biochemistry. After college, he spent five years living in South Lake Tahoe, where his love of the mountains and skiing eventually led him to discover his professional calling in podiatric medicine while working as a medical assistant for a podiatrist in the Tahoe–Carson City area.

Dr. Wallace attended the California School of Podiatric Medicine at Samuel Merritt University in Oakland, California, where he graduated as Valedictorian. He then completed an intensive three-year surgical residency at Ascension St. Vincent in Indianapolis, training in reconstructive foot and ankle surgery under more than 60 surgeons and serving as Chief Resident. His residency provided extensive experience in both complex foot and ankle reconstruction and advanced surgical techniques.

Dr. Wallace specializes in reconstructive foot and ankle surgery, with advanced training in the treatment of bunions, arthritis, traumatic injuries, Charcot deformity, hammertoes, wounds, arthroscopy, complex deformity correction, and external fixation. He also offers minimally invasive surgical techniques for appropriate conditions, including bunion correction. In addition to surgery, Dr. Wallace enjoys treating the full spectrum of foot and ankle conditions, including plantar fasciitis, warts, arthritis, flat feet, tendon disorders, sports injuries, and diabetic foot care.

A particular focus of Dr. Wallace’s practice is peripheral nerve surgery of the foot, ankle, and lower extremity. As a foot and ankle surgeon with advanced training in peripheral nerve surgery, Dr. Wallace evaluates and treats patients throughout the Phoenix metropolitan area and Arizona who suffer from chronic or unexplained nerve pain. He is an Associate of the American Association of Extremity Nerve Surgeons (AENS) and has pursued advanced training in the diagnosis and surgical treatment of lower-extremity peripheral nerve disorders.

Dr. Wallace treats a wide range of foot and ankle nerve conditions, including tarsal tunnel syndrome, common peroneal nerve entrapment, superficial and deep peroneal nerve entrapment, Baxter’s nerve entrapment, painful neuromas, Morton’s neuroma, post-surgical nerve pain, post-traumatic nerve injuries, neuropathy, and other peripheral nerve compression syndromes. Treatment is individualized and may include conservative care, nerve decompression, neurolysis, neuroma surgery, or other advanced peripheral nerve procedures when appropriate.

Many patients with peripheral nerve disorders have persistent burning, tingling, numbness, hypersensitivity, shooting pain, or unexplained foot and ankle pain despite previous treatment. Dr. Wallace has a particular interest in evaluating patients whose symptoms have been attributed to common musculoskeletal conditions but may actually have an underlying nerve component. His goal is to identify the source of the patient’s symptoms through a detailed clinical examination and determine whether a peripheral nerve treatment or surgical option may provide relief.

Dr. Wallace was appointed Associate Professor at Midwestern University’s Arizona College of Podiatric Medicine, where he serves as a dedicated lecturer in peripheral nerve surgery. He teaches podiatric medical students the evaluation, diagnosis, and surgical management of lower-extremity peripheral nerve pathology and also participates in cadaveric surgical education. In addition to classroom instruction, Dr. Wallace serves as a community clinical and surgical proctor for Midwestern University, with medical students rotating alongside him in both the clinic and operating room.

Dr. Wallace is also passionate about surgical education and regularly teaches and mentors residents from multiple residency programs in the operating room across a broad range of foot and ankle surgical procedures. His involvement in medical student and resident education allows him to remain closely engaged with evolving surgical techniques while helping train the next generation of foot and ankle surgeons. He particularly enjoys sharing his experience in peripheral nerve surgery—an area of foot and ankle medicine that can be overlooked in patients suffering from chronic or difficult-to-diagnose pain.

Outside of medicine, Dr. Wallace enjoys spending time with his wife, whom he met during medical school. She is also a podiatrist and a native of Peoria, Arizona, and together they are grateful to serve patients and build their family in Arizona near relatives and friends. They enjoy hiking, kayaking, paddle boarding, traveling, board games, family gatherings, and spending time with their two cats and two golden retrievers. They are currently expecting their first child, a daughter, in December 2026, and Dr. Wallace is especially excited to begin his newest role—as a dad.

Dr Wallace is accepting new patients and is ready to help anyone with foot and ankle pain.
